Download our free <a href=\"https:\/\/chministries.org\/maternity-guide\/\">Maternity Guide<\/a> to learn more about how CHM supports you during this exciting stage of life.<\/p>\n\n\n<section class=\"chm-block-accordion initial wp-block-acf-accordion\" id=\"block_e3ad798c233cb40eac70b19fbb9e9f0a\" >\n\n  <div class=\"accordion-item\"><div class=\"title\">Is CHM&#8217;s Maternity Care Solution available on all programs?<\/div><div class=\"content\"><p>Yes. The maternity event will be shared according to the member\u2019s program participation 300 days prior to their expected due date, as verified by a medical professional.<\/p>\n<p>Eligible maternity events must satisfy the following maternity Personal Responsibility amounts before any reimbursement funds can be distributed:<\/p>\n<ul>\n<li>CHM Gold\u2014$2,500<\/li>\n<li>CHM Silver\u2014$5,000<\/li>\n<li>CHM Bronze\u2014$9,000<\/li>\n<\/ul>\n<p>For more information about maternity Personal Responsibility and switching programs with respect to your maternity event, see the <a href=\"https:\/\/member-portal.chministries.org\/resource\/chm-guidelines\/\" target=\"_blank\" rel=\"noopener\">CHM Guidelines<\/a>.<\/p>\n<\/div><\/div><div class=\"accordion-item\"><div class=\"title\">What\u2019s eligible under the maternity solution?<\/div><div class=\"content\"><p>CHM shares qualifying medical bills for prenatal care, delivery (including cesarean and multiple births), home births, midwives, postnatal care, and complications that arise for mother and baby.<\/p>\n<p>For prenatal care, CHM shares:<\/p>\n<ul>\n<li>routine office visits<\/li>\n<li>lab tests<\/li>\n<li>maternity-related prescriptions and immunizations<\/li>\n<li>three ultrasounds<\/li>\n<\/ul>\n<p>For delivery, CHM shares costs associated with:<\/p>\n<ul>\n<li>facility labor and delivery<\/li>\n<li>physician labor and delivery<\/li>\n<li>anesthesiology services<\/li>\n<li>complications that arise for mother and baby<\/li>\n<\/ul>\n<p><strong><em>Note<\/em><\/strong><em>: CHM shares expenses from either a midwife or a physician, not both. (For an exception, see CHM\u2019s maternity Guidelines.) We cannot share expenses from more than one midwife.<\/em><\/p>\n<p>For postnatal care, CHM shares:<\/p>\n<ul>\n<li>mother\u2019s postpartum check-ups<\/li>\n<li>lactation consultations<\/li>\n<li>postpartum depression treatment<\/li>\n<li>wellness visits, circumcision, and tongue tie for baby*<\/li>\n<\/ul>\n<p>For a list of ineligible maternity expenses, please refer to CHM\u2019s maternity Guidelines<\/p>\n<p><em>*See maternity Guidelines for more information on baby\u2019s eligibility under mother\u2019s membership unit.<\/em><\/p>\n<\/div><\/div><div class=\"accordion-item\"><div class=\"title\">What can I expect after submitting my maternity incident?<\/div><div class=\"content\"><p>Once CHM receives all the necessary paperwork and invoices, we\u2019ll review the bills to make sure they\u2019re properly itemized and eligible according to CHM Guidelines. Once CHM issues your reimbursement check, you\u2019re responsible to promptly pay your healthcare provider.<\/p>\n<p>Documents required for sharing include:<\/p>\n<ul>\n<li>your prepayment agreement\/global fee<\/li>\n<li>Medical Information Release and Patient Delegation Form<\/li>\n<li>Maternity Verification Form<\/li>\n<li>itemized bills for services not included in your prepayment agreement<\/li>\n<\/ul>\n<p>Failure to submit the necessary documents can result in sharing delays. We recommend uploading via the <a href=\"http:\/\/member-portal.chministries.org\" target=\"_blank\" rel=\"noopener\">Member Portal<\/a>, as all the necessary forms are fillable directly from the Portal.<\/p>\n<p><strong><em>Please note<\/em><\/strong><em>: CHM will process your prepayment agreement near the 30th week of pregnancy. We ask that you plan on checks going out around that time.<\/em><\/p>\n<p>A prepayment agreement for facility charges must include the length of the hospital stay and whether or not the baby\u2019s facility charges are included.<\/p>\n<p>&nbsp;<\/p>\n<\/div><\/div><div class=\"accordion-item\"><div class=\"title\">What is a prepayment agreement\/global fee?<\/div><div class=\"content\"><p><!-- wp:paragraph --><\/p>\n<p>Your physician or midwife may provide you with a\u00a0<a href=\"https:\/\/6634526.fs1.hubspotusercontent-na1.net\/hubfs\/6634526\/Campaigns\/Maternity%20Support%20Team\/11_2023\/PHYSICIAN%20GLOBAL%20AGREEMENT_FINAL_newbranding.pdf\" target=\"_blank\" rel=\"noopener\">prepayment agreement<\/a>\/ global fee. Please review your agreement before sending in itemized bills.<\/em><\/p>\n<p><!-- \/wp:paragraph --><\/p>\n<\/div><\/div><div class=\"accordion-item\"><div class=\"title\">What&#8217;s the difference between a prepayment and an estimate?<\/div><div class=\"content\"><p><!-- wp:paragraph --><\/p>\n<p>An\u00a0<strong>estimate<\/strong>\u00a0is a ballpark figure that&#8217;s subject to change.<\/p>\n<p>A\u00a0<strong>prepayment agreement<\/strong>\u00a0is an agreed-upon price that stays the same as long as the conditions of the agreement are met.<\/p>\n<p><!-- \/wp:paragraph --> <!-- wp:paragraph --><\/p>\n<p>While prepayment agreements are preferable, if you receive an estimate, please submit it so our team can work with you and your provider to receive additional information.<\/p>\n<p><!-- \/wp:paragraph --> <!-- wp:paragraph --><\/p>\n<p>If you need assistance obtaining a prepayment agreement, please contact the Maternity Care Team at <a href=\"tel:1-800-791-6225\">1-800-791-6225<\/a>.<\/p>\n<p><!-- \/wp:paragraph --><\/p>\n<\/div><\/div><div class=\"accordion-item\"><div class=\"title\">Does CHM reimburse facility\/hospital prepayment agreements?<\/div><div class=\"content\"><p><!-- wp:paragraph --><\/p>\n<p>Our Maternity Care Team (MCT) wants to help our members receive bundled facility flat rates. Please contact the MST at <a href=\"tel:1-800-791-6225\">1-800-791-6225<\/a>\u00a0for assistance finding flat rates available in your area.<\/p>\n<p><!-- \/wp:paragraph --> <!-- wp:paragraph --><\/p>\n<p>Facility\/hospital prepayment agreements (or flat rates) must include:<\/p>\n<ul>\n<li>the patient\u2019s name<\/li>\n<li>provider or place of service<!-- \/wp:list-item --> <!-- wp:list-item --><\/li>\n<li>CPT codes or descriptions of services<!-- \/wp:list-item --> <!-- wp:list-item --><\/li>\n<li>the total charge for each service<!-- \/wp:list-item --> <!-- wp:list-item --><\/li>\n<li>length of your hospital stay<!-- \/wp:list-item --> <!-- wp:list-item --><\/li>\n<li>whether newborn charges are included<!-- \/wp:list-item --> <!-- wp:list-item --><\/li>\n<li>due date of the prepayment agreement<\/li>\n<\/ul>\n<p><!-- \/wp:list-item --><\/p>\n<p><!-- \/wp:list --> <!-- wp:paragraph --><\/p>\n<p>Anesthesiology charges are eligible for sharing, but CHM cannot share these expenses prior to the birth. You\u2019ll need to pay these charges out of pocket if your provider requires payment before delivery. You can upload the estimated bill into the CHM <a href=\"http:\/\/member-portal.chministries.org\" target=\"_blank\" rel=\"noopener\">Member Portal<\/a> prior to delivery, then submit the actual bill after the birth. Following this suggestion will significantly shorten sharing time.<\/p>\n<p><!-- \/wp:paragraph --> <!-- wp:paragraph --><\/p>\n<p><em><strong>Note:\u00a0<\/strong>You may receive additional medical bills for your baby\u2019s after-delivery hospital care, such as lab work, hearing tests, pediatrician visits, etc. You can submit the itemized bills as \u201cmaternity add-on\u201d bills.<\/em><\/p>\n<p><!-- \/wp:paragraph --><\/p>\n<\/div><\/div><div class=\"accordion-item\"><div class=\"title\">Is genetic testing eligible for sharing?<\/div><div class=\"content\"><p><!-- wp:paragraph --><\/p>\n<p>Genetic testing may be eligible for sharing if:<\/p>\n<ol>\n<li>It\u2019s non-invasive <strong>and\u00a0<\/strong>required to determine treatment for a current medical condition.<\/li>\n<li>Medical records or healthcare provider notes are provided for verification.<\/li>\n<\/ol>\n<p><!-- \/wp:list-item --><\/p>\n<p><!-- \/wp:list --> <!-- wp:paragraph --><\/p>\n<p><em><strong>Note<\/strong>: An amniocentesis may be considered for sharing when medical records confirm that the procedure is necessary to determine life-preserving medical care for the baby and\/or mother.<\/em><\/p>\n<p><!-- \/wp:paragraph --><\/p>\n<\/div><\/div><div class=\"accordion-item\"><div class=\"title\">Are medical bills for congenital conditions eligible for sharing?<\/div><div class=\"content\"><p><!-- wp:paragraph --><\/p>\n<p>For members whose birth was shared through CHM\u2019s maternity program, CHM shares up to $200,000 for medical bills related to their congenital condition if the following conditions are met:<\/p>\n<ol>\n<li>For a dependent child, the child\u2019s mother must be a CHM <strong>Gold\u00a0<\/strong>member with an account in good standing and must remain a CHM Gold member until $200,000 is shared.<\/li>\n<li>The individual who incurred the bills has continuously been a CHM <strong>Gold\u00a0<\/strong>member (no interruptions in membership) since birth or adoption.<\/li>\n<\/ol>\n<p>For CHM Silver and CHM Bronze members, or CHM Gold members who don&#8217;t meet the above requirements, you can submit expenses for congenital conditions for sharing with a maximum total of $25,000 per illness.<\/p>\n<p><!-- \/wp:list-item --><\/p>\n<p><!-- \/wp:list --> <!-- wp:paragraph --><\/p>\n<p><em><strong>Note:<\/strong>\u00a0Babies with a congenital birth defect must transition immediately from their mother\u2019s membership unit to their own unit.<\/em><\/p>\n<p><!-- \/wp:paragraph --> <!-- wp:paragraph --><\/p>\n<p>For more information, please reference the\u00a0<a href=\"https:\/\/member-portal.chministries.org\/resource\/chm-guidelines\/\" target=\"_blank\" rel=\"noopener\">CHM Guidelines<\/a>.<\/p>\n<p><!-- \/wp:paragraph --><\/p>\n<\/div><\/div><div class=\"accordion-item\"><div class=\"title\">How does CHM spiritually support members?<\/div><div class=\"content\"><p>We believe supporting each other spiritually is as important as the financial support members receive. Our Maternity Care Team will pray for you, provide you with spiritual resources, and encourage you to participate in a community with each other.<\/p>\n<p>CHM members lift each other up in prayer through the <a href=\"https:\/\/member-portal.chministries.org\/prayer-request\/\" target=\"_blank\" rel=\"noopener\">Prayer Wall<\/a>, located on the <a href=\"http:\/\/member-porta.chministries.org\" target=\"_blank\" rel=\"noopener\">Member Portal<\/a>. See prayer requests submitted by your fellow CHM members, and submit prayer requests of your own so your CHM family can pray for you, too.<\/p>\n<p>Through Prayers Unceasing, included in your Monthly Contribution Form, members have the opportunity to reach out to each other through prayer, cards, and notes of encouragement. By visiting us every Wednesday on social media, you can join members across the country in praying for one another.<\/p>\n<p>CHM staff also pray daily for members, especially those who have submitted prayer requests by mail, email, or phone.
